Angelle’s Hospice & Palliative Care will assign an RN Case Manager to ensure your care needs are met at home or wherever the patient calls home.
Hospice care is a team effort with the patient and family at the top of the team. The RN Case Manager works with the doctors, nurses, social workers, counselors, home health aides, clergy, therapists, and trained volunteers to coordinate the care of each patient. The RN case manager will work hard to ensure everything you need is in place including adequate staff, equipment, supplies and medications.
In addition, the RN Case Manager becomes the “eyes and ears” of the physician, communicating to the physician what is occurring, what the vital signs are, what symptoms have arisen and providing information necessary for the physician to determine appropriate medical orders for the patient.
Angelle’s Hospice and Palliative Care also provides nursing services in your own home when your loved one experiences a crisis.
