Angelle’s Hospice & Palliative Care team comes together as a family to serve you and your family. Our care team of palliative care professionals are committed to serve by bringing their expertise, training and values to each one of our patients and families. Physician: Patients can keep their personal physician. Our medical director collaborates with your personal physician to develop a plan of care, and oversees palliative care treatments.
RN Case Manager: Our Registered Nurses are trained in pain and symptom management and provide hospice care. A hospice nurse is available 24-hours a day, 7 days a week.
Certified Nursing Assistant: Hospice aides provide personal care (dressing, bathing, shaving, etc.) Their hands of service provide comfort and dignity.
Social Workers: Hospice social workers provide counseling and education to enhance patient and family coping. They are knowledgeable about financial and community resources and are a source of practical assistance.
Chaplains: Chaplains offer spiritual care in accordance with the wishes of the patient and family. Chaplains also coordinate with patients’ clergy or religious leader.
Bereavement Coordinator: The Bereavement Coordinator is trained to offer grief support through phone calls, letters, support groups, and personal visits.
Volunteer: Volunteers offer companionship. Sitting and visiting, reading, playing music, taking magazines and flowers are all examples of comfort care that volunteers provide faithfully.
